Online Hearing Screener Terms of Use and Disclosure

Please read this disclosure before beginning Nova Hearing Center’s online hearing screener. By selecting “I Agree” and using the screener, you acknowledge and agree to the following:

1. Screening Tool Only

​

This online hearing screener is provided for general informational and educational purposes. It may help identify signs that further hearing evaluation could be beneficial, but it is not a comprehensive hearing examination.

The screener and its results do not:

• Diagnose hearing loss or any medical condition

• Determine the type, cause, or severity of a hearing problem

• Replace an in-person audiometric evaluation

• Replace examination, diagnosis, or treatment by a physician, audiologist, hearing instrument specialist, or other qualified healthcare professional

• Establish a provider-patient relationship by themselves

A “pass,” “within range,” or similar result does not guarantee normal hearing or rule out an ear-related or medical condition.

2. Accuracy and Testing Conditions

​

Online screening results can be affected by factors outside Nova Hearing Center’s control, including:

• Headphone or speaker quality

• Device volume and audio settings

• Background noise

• Internet, browser, or device performance

• Incorrect placement of headphones

• Differences between the right and left audio channels

• The user’s understanding of or response to the instructions

For the most useful result, complete the screener in a quiet room using properly fitted headphones. Do not change your device’s volume after completing any calibration step.

Nova Hearing Center does not guarantee that the screener or its results will be accurate, complete, or suitable for diagnosing or treating any condition.

3. Results and Recommended Follow-Up

​

Your result is an initial screening result—not a diagnosis or prescription. If the screener identifies a possible concern, or if you experience difficulty hearing despite the result, schedule a comprehensive hearing evaluation.

A hearing professional may need to perform additional testing to evaluate hearing sensitivity, speech understanding, middle-ear function, and other aspects of your hearing health.

4. Medical Warning

​

This screener is not intended for emergencies or urgent medical concerns.

Seek prompt medical attention if you experience:

• Sudden hearing loss in one or both ears

• Hearing that decreases rapidly over several hours or days

• Severe dizziness or new balance problems

• Ear pain, bleeding, or drainage

• Hearing loss following a head or ear injury

• Ringing or pulsing that occurs in only one ear or follows your heartbeat

• Facial weakness, numbness, severe headache, confusion, or other neurological symptoms

• A foreign object in the ear

Sudden hearing loss may be a medical emergency. Do not wait for an online screening result or routine hearing appointment. Contact a physician, an ear, nose, and throat specialist, an urgent care center, or emergency services as appropriate.

5. Children and Minors

​

This screener is intended for adults unless Nova Hearing Center expressly states otherwise.

Anyone under 18 should use the screener only with the involvement and consent of a parent or legal guardian. Online screening should not replace pediatric hearing testing or medical evaluation when there are concerns about a child’s hearing, speech, language development, school performance, or ear health.

 

6. Privacy and Communications

 

Information submitted through the screener will be handled according to Nova Hearing Center’s [Privacy Policy] and, when applicable, its [Notice of Privacy Practices].

 

Before submitting personally identifiable or health-related information, please review those notices to understand:

• What information is collected

• How that information may be used

• Whether it may be shared with service providers

• How long it may be retained

• Your available privacy rights

• How to contact Nova Hearing Center with a privacy question or request

Submitting the screener does not automatically authorize marketing calls, text messages, or emails. Any optional consent to receive promotional communications must be presented separately and is not required to complete the screener or receive healthcare services.

If you request an appointment or follow-up, you authorize Nova Hearing Center to contact you using the information you provide for that purpose. Standard telephone, text-message, or data charges may apply.

7. Technology and Availability

​

Nova Hearing Center may modify, suspend, or discontinue the screener at any time. The screener may not function correctly on every device, browser, headphone system, or assistive technology.

To the extent permitted by law, the screener is provided “as is” and without warranties regarding uninterrupted availability, accuracy, or fitness for a particular purpose. Nothing in this disclosure limits rights or remedies that cannot legally be waived.

8. Proper Use

You agree to:

• Provide information that is accurate to the best of your knowledge

• Follow the testing and calibration instructions

• Use the screener only for its intended personal purpose

• Avoid copying, modifying, disrupting, reverse-engineering, or misusing the screener

• Avoid using another person’s identifying information without authorization

9. Changes to This Disclosure

Nova Hearing Center may update this disclosure to reflect changes in its services, technology, or legal obligations. The effective date shown above identifies the most recent version. Your use of the screener is governed by the version presented when you begin the screening.
